The Prevention Plan™
Enables individuals to determine their top health risks and receive a customized plan and coaching from nurses to lower those risks and become healthier.
The Prevention Plan CMSM
Brings intensive disease management resources to individuals with costly chronic conditions. By understanding the barriers that impede better health and working closely with each person and his/her health care providers - either telephonically or in the home and workplace-Prevention Plan RNs work to limit the progression of an existing condition and detect other potential issues before they cause harm.
The Prevention Plan PremiumSM
Provides early disease detection and health management program for executives and individuals. Available through the Global Prevention Network™, U.S. Prevention Network™, and The Center for Preventive Medicine®, members receive early disease detection assessments through advanced diagnostic technology as well as needed follow up and specialty care.
The Prevention Plan PrimeSM
In close collaboration with the medical staff at the Senior Care Group communities, our RN Advocates provide Prime members with thorough, long-term preventive services such as health risk assessments, personal coaching and resource coordination. Other member benefits include having a personal liaison with family and communities services as well as lifestyle improvements related to key concerns such as safety, diet and exercise.
The Global Prevention Network™
Provides an international network of preferred providers to offer preventive and executive health programs, with follow-up care, to The Prevention Plan™ members.
U.S. Prevention Network™ Provides Prevention Plan members with a medical home for state-of-the-art preventive services and follow-up care, as indicated by their personal prevention plan.
The Center for Preventive Medicine®
Provides early disease detection assessments through advanced diagnostic technology. Each Center combines the resources of a physician group, hospital, and U.S. Preventive Medicine in a business arrangement that provides branded preventive medicine services exclusively to a local community.
